Habitat: larvae found in marshes with dense vegetation

General appearance: body medium sized, up to about 15mm long; head pentagonal with two pairs of dark spots; pale, broad stripe on midline of abdomen; three caudal gills broad, leaf-like with many dark spots scattered throughout the gills
